Transcripts for Penn Dental Medicine graduates of class years 2000 and after are generated by the Office of the University Registrar by mail (to the address below) or in person. The Office of the University Registrar does not accept e-mail, fax or phone requests for transcripts.

Transcripts for Penn Dental Medicine alumni who graduated prior to 2000 are generated by the Office of Student Affairs at the School of Dental Medicine. A written request must be submitted, accompanied by a fee of $10 per transcript. Overnight express shipping is available for an additional fee of $15 per mailing. Transcript requests for alumni who graduated before 2000 may be e-mailed to Office of Academic Affairs, faxed to 215-898-7506 or mailed to the address below. Payment must be made by mail and will only be accepted in the form of a check or money order made out to The Trustees of the University of Pennsylvania, School of Dental Medicine.

Information required for the transcript request includes:

  • Full Name (as it appears on your student record)
  • Date of birth
  • Dates attended
  • Degree received
  • Number of transcripts requested
  • Address(es) where the transcripts are to be sent
  • E-mail address and daytime phone number where you can be contacted

(please do not submit your Social Security Number)
