The Penn Faculty Practice Dental Plan for University of Pennsylvania Health System Employees - General Information

How Does the Plan Work?
It's simple. Once you become a subscriber, all you need to do is call the office of your choice (three convenient locations) when you are ready for a check-up or have a problem and you will be scheduled with a dentist of your choice. When you arrive for your appointment, simply give your name to the receptionist - no ID Cards or claims forms are required to receive care under the Penn Faculty Practice (PFP) Dental Plan.

What is Covered?
The PFP Dental Plan for University of Pennsylvania Health System (UPHS) Employees provides full coverage for preventive and basic restorative services and coverage at substantially reduced rates for other services. There is a maximum benefit limit of $3,000 per plan year per person covered.

Type I Services - 100% Coverage
Includes routine examinations, cleanings, fluoride applications, sealants, and other diagnostic services.

Type II Services - 100% Coverage
Includes basic restorative fillings and extractions.

Type III Services - 80% Coverage
Includes subgingival curettage, root planing, scaling, surgical periodontics, pulp treatment, root canal therapy, and apicoectomy.

Type IV Services - 50% Coverage
Including inlays, onlays, crowns (when determined by the dentist to be necessary), bridges, dentures, and space maintainers.

Type V Services - 50% Coverage
Includes orthodontic coverage for children under the age of 19 only ($2,000 lifetime maximum benefit).

Emergency Care - 100% Coverage
Emergency treatment to relieve pain, fever, swelling, bleeding, or severe discomfort or for tooth loss is available on a 24-hour basis.

Will I Be Treated by a Penn Dentist?
Yes. All treatment is provided by faculty and staff of the University of Pennsylvania School of Dental Medicine. In addition to general practice dentists, care is provided by periodontists, who specialize in the treatment of gum disease; endodontists, who specialize in treating teeth with nerve damage; and prosthodontists, who specialize in complex restorations. There are also specialists in oral surgery and implantology.

You may receive care at any of the office locations of the University of Pennsylvania Dental Care Network and change offices at any time during your treatment:

Is There a Deductible?
For Type II, Type III and Type IV services, a deductible of $50 applies. After the deductible, Type II Services are covered at 80%; Type III and Type IV Services are covered at 50% of the remaining cost of treatment. The maximum deductible is $50 per individual per year, or $150 per family per year. For all services, you will be asked to make your copayment, if any, at the time of your visit. Arrangements can be made for periodic payment of lump sum copayments, such as those for crown and bridge work.

Who Is Eligible for Coverage?
The PFP Dental Plan is open to employees of the University of Pennsylvania Health System. For eligibility provisions, employees should refer to their enrollment materials provided by the UPHS Office of Human Resources.

How Do I Enroll?
UPHS employees may enroll in the PFP Dental Plan during the annual open enrollment period. Eligible new employees may enroll upon hire.   Outside of open enrollment or the new hire period, you may not make changes to your benefit elections until the next open enrollment unless you experience a Qualified Life Event, such as marriage. You have 30 days from the date of that event to make a change.  Please refer to the enrollment materials provided by the UPHS Benefits Office for additional information about making changes during the year.
