Get information on requesting a transcript, requesting a replacement diploma, and verifying education credentials.

 

Transcripts for all Penn Dental Medicine graduates are generated by the Office of the University Registrar. The Office of the University Registrar does not accept e-mail, fax or phone requests for transcripts. Transcripts can be requested through this link.

Please note that the Office of the University Registrar is undergoing a redesign. For all Penn Dental Medicine graduates, you will need to select the option on the Office of the University Registrar page that says “Alumni of Dental School prior to 2000”

In the event that a diploma is lost or destroyed, replacement requests must be made directly to the Office of the University Secretary. The replacement cost is $75-$100. Replacement and duplicate diplomas take eight to twelve weeks to produce.

The Office of the University Secretary also offers certified translations of the diploma upon request, without charge. These documents provide the English text of the Latin appearing on diplomas.

Verification of Education for Employers: Employers wishing to verify the education of a Penn Dental Medicine graduate must submit their request verification AND a release signed by the student via our Credentialing Request Form, mail (to the address below), or scan/e-mail to Office of Academic Affairs . Verifications cannot be completed without the signature of the alumna/us. Please include the graduate’s name at time of attendance. No fee will be assessed.

Alternatively, employers wishing to verify the education of a graduate from 2000 or later may contact the National Student Clearinghouse. A fee will be assessed.

Verification of Attendance for DMD Alumni: Alumni requiring verification of attendance, listing the degree award date, degree received, and dates of attendance should complete our Credentialing Request Form, mail (to the address below), e-mail Office of Academic Affairs, or fax their request to 215-898-7506. Please list your full name (as it appears on your student record), contact information, date of birth, degrees received, date of attendance, and addresses to which the letter(s) should be sent. This letter is written on official School letterhead, contains the signature of the Dean, and is embossed with the seal of the School of Dental Medicine. No fee will be assessed.

The Office of the University Secretary offers a notarized letter of certification upon request. Notarized letters of certification attest to the name of the graduate, the degree conferred, and the date of conferral. They are notarized and signed by the Secretary, and embossed with the seal of the University. A fee will be assessed.

Verification of Attendance for Residencies: Individuals who attended the School of Dental Medicine as residents and require certification of attendance should complete our Credentialing Request Form, mail (to the address below), e-mail Office of Academic Affairs, or fax their request to 215-898-7506. Please list your full name (as it appears on your student record), contact information, date of birth, residency program attended (i.e. Periodontics), date of attendance, and address(es) to which the letter(s) should be sent. This letter is written on official School letterhead, contains the signature of the Dean, and is embossed with the seal of the School of Dental Medicine. No fee will be assessed.

Verification of Nitrous Oxide Certification: Individuals who require proof of completion of the Nitrous Oxide Certificate course should complete our Credentialing Request Form or e-mail Office of Academic Affairs in the Office of Academic Affairs at Penn Dental Medicine or call 215-746-1459. Please allow ample time for this particular verification.

National Board Scores: National Board Scores are available through the American Dental Association (ADA) and are not stored at the School of Dental Medicine after graduation. To locate the order form for National Board score reports, vist the Joint Commission on National Board Dental Examinationsweb page; select Part I, Part II, or Dental Hygiene; and follow the instructions for Step 5.
